REACH (Realizing Everyday Achievements Can Happen)

ICF/IID – Intermediate Care Facility for Individuals with Intellectual Disabilities.

The Reach Corporation is a private, non-profit 501(c)(3) residential agency serving individuals with mild, moderate, severe or profound intellectual disabilities, since 2008. Reach serves 128 developmentally disabled clients. The homes are located in three different towns within Oklahoma, Okemah, Okmulgee and Weleetka. Each area has 6 and 8 bed facilities, either male or female homes.

Each area includes a day program, which provides clients with vocational and pre-vocational skills to prepare them for employment in the community.

Age 18 and up

Individuals at least 18 years of age may be admitted to The Reach Corporation Homes.

24/7 Care

Homes are staffed 24/7 by a number of highly qualified individuals.

Technology

Technology is used to assist individuals in reaching their full potential.

About The Reach Corporation

Vision

To consistently provide outstanding services, while focusing on continuous quality improvement of existing services and simultaneously aggressively expanding services and opportunities.

Admission

Individuals 18 years of age and older that have been diagnosed with Mild, Moderate, Severe or Profound mental retardation may be admitted. Individuals must have a current psychological evaluation within the last year, with an IQ of 70 or below. Must have Medicaid services.

If Reach can provide services, information will be requested. An in person screening would be conducted, and if Reach can meet the needs of the individual a placement would be offered.

Funding

The Reach Corporation receives funding from the Federal Government’s Medicaid program and is classified as an Intermediate Care Facility for Individual’s with Intellectual Disabilities (ICF/IID).

The Homes

Individuals live in modern homes with living room, dining area, kitchen, laundry room, bathrooms, bedrooms, and safe room (designed to with stand an F4 Tornado). Either 6 or 8 individuals live in each home under the care of a House Manager, Certified Medication Aide, and Direct Care staff 24/7.
Each area has a Licensed Nursing Home Administrator, Director of Nursing, QIIDP (Qualified Individual with Intellectual Disabilities Professional).

Management

The Reach Corporation is managed by a group of specialist under the direction of the Executive Director. Each regional area is managed by a Licensed Nursing Home Administrator, a Licensed Nursing Practitioner (LPN), and a Qualified Individual with Intellectual Disabilities Professional (QIIDP).

Training

The Reach Corporation hires non certified people part-time as trainers at minimum wage, while attending DDCA class (Developmental Disability Care Aide). Trainers will be offered DDCA positions once they have become certified. For more information, contact the Central Office for next scheduled class.
Reach has been approved by the Oklahoma State Department of Health to conduct classes to help people get their DDCA certification. People that qualify, and are accepted into the class, will be hired and paid during the class time. The class will last 4 weeks, Monday – Friday. Classes will be held in Henryetta, OK.
